Lisbon has an extensive public and private healthcare network, but access is not identical across the city. A short distance on a map can become a much longer journey because of traffic, hills, parking or public transport connections.

Many foreign residents use a combination of the two. They may rely on the SNS for primary care, hospital treatment or long-term monitoring while using private providers when they want faster scheduling, a particular doctor or more predictable appointment times.

This does not mean that one system must replace the other. For many residents, the practical solution is to use public and private healthcare according to the situation.

Does Buying Property Give You Access to the SNS?

No. Property ownership and healthcare entitlement are separate matters.

A foreign citizen may be assigned a Portuguese national health user number, known as a número de utente, when first attending a public healthcare unit. However, the Portuguese government states that receiving this number does not, by itself, guarantee that the SNS will cover the cost of treatment.

Coverage depends on the person’s circumstances and the information associated with their registration. This may include identification, a Portuguese tax number, a Portuguese address and valid residence documentation.

Foreign citizens with an address in Portugal who meet the relevant conditions may also be able to request their tax, Social Security and national health identification numbers through the combined government service for foreign residents.

Purchasing a home may support a relocation plan, but the property deed is not a healthcare entitlement. Buyers should confirm their position before moving, particularly when regular medication, treatment or specialist appointments are involved.

Lisbon’s Main Public Hospitals

Lisbon’s public hospital network is now organised mainly through Local Health Units, known as Unidades Locais de Saúde. These structures combine hospitals and primary healthcare services within defined geographical areas. The national restructuring that created the current ULS model came into effect through Decree-Law 102/2023.

ULS Santa Maria resulted from the restructuring of the former Centro Hospitalar Universitário Lisboa Norte, while Hospital Pulido Valente continues to operate services within this ULS.

ULS São José replaced the former Centro Hospitalar Universitário Lisboa Central and integrates several historic hospital units across central Lisbon. ULS Lisboa Ocidental includes São Francisco Xavier, Egas Moniz and Santa Cruz.

Hospital de Santa Maria

Hospital de Santa Maria is one of Lisbon’s main public hospitals and is especially relevant for residents of Alvalade, Campo Grande and Telheiras.

Its location beside the University of Lisbon and access to the Metro can make it practical for residents who do not drive. Nevertheless, buyers should test the complete journey from the property rather than relying only on straight-line distance.

ULS São José and Central Lisbon

The hospitals within ULS São José cover several central locations and specialist services. Hospital de São José is near Martim Moniz, while Dona Estefânia is particularly associated with paediatric care and Santa Marta with cardiovascular treatment.

For buyers considering Arroios, Avenidas Novas or Areeiro, this network can be relatively convenient. Traffic, parking, hills and the route from the property should still be tested in realistic conditions.

ULS Lisboa Ocidental

ULS Lisboa Ocidental includes Hospital de São Francisco Xavier in Restelo, Hospital de Egas Moniz in Junqueira and Hospital de Santa Cruz in Carnaxide.

This network is particularly relevant to buyers considering Belém, Restelo, Ajuda, Alcântara or western Lisbon. Egas Moniz and São Francisco Xavier are within the municipality of Lisbon, while Santa Cruz is in nearby Carnaxide.

Lisbon’s Main Private Hospitals

Private healthcare is widely used by foreign residents, particularly for routine consultations, diagnostic tests, elective procedures and situations where appointment speed matters.

Hospital CUF Tejo replaced the former Hospital CUF Infante Santo in 2020. The CUF network currently operates both CUF Tejo and CUF Descobertas as major Lisbon hospitals, as well as smaller clinics elsewhere.

Hospital da Luz Lisboa is located on Avenida Lusíada, while Hospital Lusíadas Lisboa is situated in Alto dos Moinhos, close to the Metro’s Blue Line.

Always check a private hospital against the intended insurance policy. An insurer may have an agreement with a hospital, but conditions can vary according to the policy, doctor, procedure or type of treatment.

The main differences are usually waiting time, provider choice, administration and cost.

For registered and eligible users, the SNS offers broad access to public care. Waiting times for non-urgent consultations, examinations or procedures can, however, be longer than in the private sector.

Private care generally offers faster booking and more choice of doctor, but treatment must be paid for directly or covered under an insurance policy, health plan or other agreement.

English is spoken by many healthcare professionals, particularly in large private hospitals and major Lisbon institutions. Nevertheless, patients should not assume that every department or shift will provide service in English.

People with complex conditions may benefit from bringing translated medical records, recent test results and a clear list of medication.

For families, access to paediatrics, maternity care and urgent treatment may be more important than proximity to a general clinic. Hospital Dona Estefânia is an important part of Lisbon’s public paediatric network, while the main private hospital groups provide paediatric services with different schedules and conditions.

Anyone relocating with a chronic illness should first identify the relevant specialist service and only then compare neighbourhoods. The nearest general hospital may not be the most appropriate institution for a particular condition.

Healthcare should rarely be the only reason for choosing a property, but it can be an important reason for rejecting one.

A home may appear close to a hospital on a map and still involve a difficult journey during rush hour. Another neighbourhood slightly farther away may offer a direct Metro route, easier parking, a newer building with a lift and better access to pharmacies and primary care.
